This task is to create the UI setup for the State of Illinois GLTG Instance of Geodashboard.

Per discussion with jonglee:

• This task should be priority over other GLTG tasks for right now
• This should try to have a similar look-and-feel to http://www.epa.illinois.gov/topics/water-quality/watershed-management/excess-nutrients/nutrient-loss-reduction-strategy/index
• This should try to use the same icon (he said this is allowed)
