Symptom: Basic auth password and htpasswd for kube-system account are out of sync, preventing login to the LMA tools via ingress.

Cause: At some point, an invalid account was created in etcd (/ndslabs/accounts/services). There are a few ways this might have happened and I'll need to investigate. As a result, an error is thrown during admin account creation and the basic-auth secret is never created or updated.
